Primecare Home Care proudly supports families across Georgia with pediatric skilled nursing services through the Georgia Pediatric Program (GAPP)—a Medicaid-based program for medically fragile children and those with physical or functional disabilities.

What Is GAPP?

The Georgia Pediatric Program (GAPP) delivers no-cost home health care for eligible children under 21 with complex medical needs. Services are fully covered through Georgia Medicaid, helping families receive essential care without financial strain.

Services We Provide

Skilled Nursing Services

 Our licensed pediatric nurses are trained to care for children with:

  • Trachs & ventilators

  • G-tube feeding

  • Ostomy care

  • Chronic and complex medical conditions

Personal Care Assistance (PCA)

 Up to 40 hours per week of non-medical support:

  • Bathing & dressing

  • Feeding assistance

  • Light housekeeping

  • Companionship

These services are designed for children ages 5 to 20 years and 11 months and can significantly reduce the need for hospital visits.

Who Is Eligible?

Children may qualify for GAPP if they:

  • Are under 21 years of age

  • Have straight Medicaid

  • Have a physical or functional disability, or are medically fragile

Note: Autism and behavioral disorders alone do not qualify for GAPP—but support is available through Primecare ABA Therapy.
